Having a discussion about end-of-life care and pre-planning for the future

It's one of the most important conversations a family can have, yet so many families fail to have it until its to late.  Having a profound discussion about end of life care takes no small amount of courage in looking ahead to the end of life.  Yet, families seem to get caught off guard when a situation comes about that those decisions need to be addressed and that time can become very stressful.  Especially now as we approach another holiday season families will gather for their holiday traditions and the subject if brought up seems to put fear in everyone involved.  Having a directive and plan in place for the envitable for all us helps those who are to handle the final decision making feel less stressful and know what their loved ones wishes were.
